No Contract for Lab Work Is Formed Here

Submitting the intake form is a request for contact, not an order for lab services.

No agreement for lab fabrication, pricing, or case acceptance is formed by using this website. Lab services, pricing, and case acceptance are agreed separately, case by case, directly with the lab. This website does not accept cases, digital files, physical impressions, or payments.
